Living at Home

Dignicare’s Living at Home services provide seniors all the help they need to stay independent in their home. We provide seniors and their caregivers a range of services. We can help with simple things like running errands or major projects like reorganizing the home to make it more accessible. Let us know what you need and we will get to work for you.

Long Distance Care Coordination

Being a long distance caregiver is difficult. Allow Dignicare to be your local eyes and ears. Our staff consisting of registered nurses and trained caregivers who will conduct regular check-ins with your loved ones to make sure they are doing well. We are on-call 24 hours a day, seven days a week. You will receive weekly communication updates about your loved one.

Everyday Help

Dignicare provides a wide range of personal care services to seniors and their caregivers. Our trained “Senior Friends” are ready to help around the house, to run errands or just to be there to provide companionship. Below is a short list of the services Dignicare provides.

Home Reorganization

To make the home safe and livable, Dignicare can rearrange the home to meet the needs of the senior. Our staff can perform a full assessment of the home to identify possible slipping and falling hazards. Based on the assessment, recommendations will be provided on how to make the home safe and livable. Dignicare will ensure the proper changes are made throughout the home.

Home Up-Keep

Dignicare has a handyman on staff that is ready and capable to make the little changes to your home. Dignicare’s handyman can install a grab bar in the bathroom or fix a leaky faucet in the kitchen. We can make changes to the home to reduce the possibility of accidents and make it safer for the senior. Dignicare is dedicated to making the necessary changes to the home so the senior can live independently for as long as possible.

Clutter Busters!

Sometimes a little organizing can go a long way. To help families, Dignicare provides home organizing and clean-up services. We help seniors and their families identify the treasures that should be kept and we find new homes for unwanted items. Our handyman can install shelves and organizing units to make the best use of your space.

Project Management

Dignicare is here to help seniors and their families with making large changes to the home. We offer project management services to help families work with the contractors when making changes to the senior’s home. Upon your request, Dignicare will identify potential contractors, screen them and secure estimates. After you accept a bid and the work is started, Dignicare will work to ensure the work is done according to schedule.

 

Senior Answers

Dignicare’s Senior Answers services are designed to provide seniors and their families the information needed to make the best decisions regarding their care. Our staff is ready and able to help provide information and find the resources needed to keep seniors independent and be a resource to caregivers.

Information

Dignicare offers seniors and their families’ assistance with:

• Choosing an Assisted Living Facility
• Selecting a Nursing Home
• Education on Medicare: Medicare HMO and Medicare Part D (prescription drugs)

Dignicare’s professional staff is ready and able to answer your questions about assisted living facilities, nursing homes and Medicare. Our staff will sit with you and your family to answer all of your questions. We will research your issues and provide recommendations that we believe best meet your needs.

Senior Care Assessments

Dignicare’s Care Advisors are available to assist families by providing comprehensive geriatric care assessments to gain a better understand of the elders social, medical and financial situation. Information collected in the assessment will be used to create a care plan. The care plan will address the senior’s issues and provide information on how to find the needed resources.


Dignicare’s Care Advisors can:

  • Perform comprehensive assessments to identify issues, and the need for services.

  • Screen, arrange, and monitor services for the senior.

  • Act as a long distance liaison for families who live out of the area.

  • Provide information and referrals for needed services.

  • Follow-up on arranged care to ensure seniors are receiving proper care.

  • Offer crisis intervention and care management services


Senior Transitions

If it is time for the senior loved one to move into a new living arrangement, Dignicare can help. We work with the professionals involved in the moving process to make sure the move is successful for the senior. Dignicare can help families identify a realtor, the moving company and the next living arrangement. Dignicare has lists of senior apartments, assisted living facilities and nursing homes in Genesee, Lapeer, Livingston and Oakland counties. We can make calls to check availability and provide information about the potential residences to save you time.

Dignicare will be by your side during packing. We will secure boxes and other moving materials for you before the move date to save time.

Dignicare will go the extra mile to ensure family heirlooms stay in the family. We will ship items to the proper destination.

Item Disposal

Dignicare will make sure the unwanted items find a new home. We can help with selling items or donating items to charity.

Coordinating the Move

  • Dignicare can identify a moving company for you. We will secure bids from each company and then provide you the information.

  • After selecting a moving company, Dignicare will work with the movers to make sure the moving process runs smoothly.

New Home Set-Up

  • The moving process is not finished until everything is unpacked and set-up.

  • Dignicare will make sure all the items are put in their proper location.

  • If requested, we can ensure that utilities (cable, telephone, internet) are ready to go on day one.
